Become a Juice Plus+ customer and buy capsules and/or chewables.
Participate
Fill in your contact information below to sign up. Qualifying customers* will receive Juice Plus+ Fruit Blend and Juice Plus+ Vegetable Blend capsules or chewables for your child—free of charge—for up to four years as a study participant.
Evaluate
After the first four to eight months of taking Juice Plus+, you will be asked to complete a simple survey on how you and your family’s health habits have been impacted. Follow-up surveys will be sent annually to track your progress.

*Requirements
Adults must be over the age of 18 and play an active role in a child's life (as a relative, mentor or godparent) to sign up as a Juice Plus+ customer. Children must be between the ages of 4 and 18 or a full-time undergraduate college student in a four-year degree program.
